We will complete any sport, camp or daycare form as long as the yearly physical is current and meets the requirements of the form. There is a small fee of $10.00 per form for the form completion unless it is completed during the annual physical. Please allow us 3-5 days to complete your form. A form that is required on the same-day is $15.00 per form (special note: due to increased demand for “same day” forms, pick-up may have to be later in the same day).
Please be aware that WPIAL requires that physicals are completed after June 1st, which will then be valid for the entire following school year. Keep in mind that your school or sports organization may require more frequent physicals. Always check with your athletic director or principal or the PIAA website for current requirements. If the yearly physical does not meet the needs of the form, a mini- physical can be scheduled to complete the form. This physical will not be submitted to the insurance company and there will be a $30.00 charge due upon the time of the visit.
Please note: Any portion of a form that requires parental/guardian information MUST be completed prior to requesting practitioner completion and signature.
We are very proud to have a team of nurse practitioners and nurses who specialize in the care of children with developmental, behavioral and mental health disorders. We recognize that approximately 21% of children and adolescents of children in the United States meet the diagnostic criteria for having a mental health disorder and have evidence of impairment. Approximately 9.5%-14.2% children from 0-5 years have developmental/behavioral problems that interfere with their function.
In order to address the needs of children and teens with behavioral and mental health disorders in a comprehensive manner, Pediatrics South has developed a system to care for children and teens with mild to moderate disorders. We recognize that performing evaluations and developing treatment plans requires a comprehensive intake assessment, longer appointment times and the use of screening tools. Therefore, the schedule used by our team allows for longer appointments and provides the ability to do close follow-up care. We only see patients from our practice to insure continuity. Our goal is to provide family centered, coordinated and ongoing care to the children and families we see. The team members are Susan Van Cleve DNP, CRNP, PMHS, Rosanne Levine, MSN, CRNP, Brittney Sabo, RN and Amber Engleman, MA, who work closely with Scott Tyson, MD and Amy Gosling, MD as collaborating physicians.
The team members have many years of experience in working with children with developmental, behavioral and mental health disorders. The types of children and teens we see are those with concerns about developmental delays, autism spectrum disorder, disruptive behavior, attention deficit hyperactivity disorder (ADHD), anxiety, depression, or Tourette’s disorder. We refer children or teens to our colleagues in psychiatry, counseling services or to pediatric or adolescent subspecialists in the community as needed. If you would like an appointment, please call 412-561-7541 extension 1120.
When you arrive at the hospital of your choice to deliver your baby, you will inform them that you will be coming to our office (Pediatrics South) with your child. You want to make sure to call our office as soon as the in-house pediatrician instructs you to (usually 3-5 days after baby is born). We can then register your baby and schedule your first appointment!
We look forward to meeting you and caring for your family for the years to come!
